  ComCare

Catholic Social Services is the administrative agency responsible for operating ComCare in Champaign, Darke, Logan, Miami, Preble, and Shelby counties. The program is provided by PSA-2, Area Agency on Aging in conjunction with the Ohio Department of Aging using Title III Funding and Senior Community Service Block Grant Funds.

Available services include case management, personal care aide / respite care, homemaker, home-delivered meals, emergency response system, adult day care, medical equipment / supplies, chore services, minor home modifications, independent living assistance, social work counseling, and transportation.

Eligibility: Residents of Champaign, Darke, Logan, Miami, Preble, and Shelby counties, age 60 and older, who are not eligible for PASSPORT. An income or resource limit is not required; however, an open slot must be available in the county of residence. Participants must have a Protective Level of Care.
