PASSPORT (Pre-admission Screening System Providing Options and Resources Today) is a home-care program funded by Medicaid that provides long-term care services to assist people over 60 in maintaining their independence. Individuals at risk of nursing home placement can often remain in their own homes with the supportive services of the PASSPORT program. Services include personal care assistance, homemaking, home-delivered meals, emergency response systems, adult day care, home medical equipment, minor home modification, medical transportation, chore services and social work counseling. An assessment at no cost can be completed for anyone interested in home care services regardless of program eligibility.

PASSPORT Providers must be certified to provide services. Interested providers should contact the CSSMV Sidney office for an application.

ComCare provides services such as case management, personal care aide / respite care, homemaker, home-delivered meals, emergency response system, adult day care, medical equipment / supplies, chore services, minor home modifications, independent living assistance, social work counseling, and transportation for eligible seniors.

Caregiver's Support Program provides help for caregivers who need assistance in providing home-care services for their loved ones. The services are personal care services, adult day services, emergency response systems and case management services.

Residential State Supplement program provides a cash supplement program for low-income adults who are aged, blind, or disabled and live in approved adult-care facilities.

Alzheimer’s Respite Care is available for people of any age with Alzheimer's or related dementia diagnosis. Available respite services are personal care services, adult day services, and adult day service transportation.

Counseling for individuals and couples is available. Counseling can address issues such as premarital relationships, marital and couple relationships, parenting skills, family relationships, stress, adapting to change such as divorce, grief, pregnancy, or loss of job, changing problematic habits, decision making, changing lifestyles, self esteem, communications skills, sadness or anxiety.

Pregnancy Counseling provides caring, confidential help for women facing an unplanned pregnancies. Complete adoption services are available. Assistance is available 24 hours / 7 days a week.

Adoption Services for domestic and international programs are available. CSSMV also offers foster care and post legal adoption programs.

Prevention / Educational Programs are available addressing a wide variety of topics. These programs emphasize that preventing problems is preferable to treating them.
