PASSPORT (Pre-admission Screening System Providing Options and Resources Today) is a government-funded program that links elderly individuals needing long-term care with the most appropriate services to meet their needs. Persons at risk of nursing home placement frequently can remain in their own homes with support services such as personal care, homemaking, home delivered meals, and medical equipment. Licensed professional PASSPORT staff assesses clients' needs, makes recommendations, and arranges for needed services. In-home services are provided through contracted agencies at no cost to Medicaid-eligible individuals 60 years of age or older. Catholic Social Services is the Administrative Agency responsible for operating PASSPORT in Champaign, Darke, Logan, Miami, Preble, and Shelby counties.
