Provider Qualifications

Any individual or agency currently operating and providing a service(s) that is used by the PASSPORT program, or a like service(s), may apply for certification. Applicant must currently serve clients in the community, has service specific qualified staff, either directly or through contract, and has written materials supporting compliance with policy, procedural, and documentation requirements of the PASSPORT Conditions of Participation (COPs) and Service Specifications (SPECS).

Conditions of Participation (COPs) and Service Specifications (SPECS)

PASSPORT COPs and SPECS are the standards by which all PASSPORT services must be delivered. The COPs apply to all service providers. The SPECS define and set the standards for individual services of PASSPORT and apply only to Providers of those services. Each Provider planning to participate in PASSPORT service delivery must read and develop an understanding of the expectations defined in the COPs and in the relevant SPECS.

Quality Assurance Process

To assure quality service and client Provider choice, PASSPORT has developed a systematic quality assurance monitoring system. The PASSPORT Quality Assurance Process is based on guarantees made to HCFA by the state of Ohio to obtain the PASSPORT Medicaid Waiver. Those guarantees include safeguards for the client's health and welfare and Provider procurement and monitoring systems. COPs and SPECS are the PASSPORT standards for Provider service delivery by which each Provider's service quality will be evaluated.
