Obtaining your Social/Medical History

Ohio Law considers all adoption files sealed at the time an adoption is finalized. However, CSSMV has permission to release non-identifying social/medical information to adult adoptees, adoptive parents of minors, and to birthparents upon their request. In order to obtain this information, please call 1-800-300-2937.

Fees for this service are based on a sliding-fee scale and range from $10.00 to $70.00. This service fee includes the initial contact and a written social/medical history.

Adoptions Finalized Before 1964

If your adoption was finalized before January of 1964, you may be entitled to receive identifying information about your birth family through Probate Court. You may also be able to receive your original birth certificate. Contact Montgomery County Probate Court at 937-225-4624 for more information.

***Please remember that Ohio law prohibits us from releasing any identifying information (names, addresses, phone numbers, social security numbers, etc.).

Search/Reunion Counseling

CSSMV will provide counseling for all members of the adoption triad as they prepare to search for birthfamily members. In Ohio, it is necessary to hire a licensed private investigator to help locate the birthfamily members. CSSMV can provide a referral for a licensed private investigator.
